I UNDERSTAND and agree that the Olivera jiujitsu & fitness academy LLC has informed me of all the rules and regulations of the establishment as well as the possible risk of injury due to the rigorous nature of Martial Arts which I may sustain in connection with any practice, class, sparring session, athletic event, or traveling to and from such endeavors. No one can assure the student/member that injury will not occur participating with/at Olivera jiujitsu & fitness academy LLC.
I UNDERSTAND and agree to not attend if I have any sickness/infections or symptoms connected to coronavirus in efforts to help stop the spread.
I UNDERSTAND The risk of attending this public facility and the risk I’m taking of Coming in contact with coronavirus (COVID-19). I understand I may catch this illness and spread it to others.
I UNDERSTAND that my tuition is arranged to be made in monthly installments and is not affected by my lesson schedule and/or attendance. I further understand that failure to complete the lessons does not relieve me of my obligations to pay the tuition in full.
I UNDERSTAND that Day camp fees are not included in tuition and will be charged separately.
I UNDERSTAND and will comply with all the rules and regulations of the school and the traditions of Martial Arts.
I UNDERSTAND and AGREE that any photography and videography taken during attendance to any classes, events etc. maybe used on social media and/or for promotional purposes. Therefore, I do hereby for myself/ my heirs/ executors/ administrators/ assigns waive, release and forever discharge any and all rights/claims.
THE UNDERSIGNED hereby represents that the student is up to date on all physicals & physically healthy to receive and participate in the prescribed course of Martial Arts instruction at OLIVERA JIUJITSU & FITNESS ACADEMY LLC.
I ACKNOWLEDGE receiving a copy of this agreement.
